Patient Intake Form
Getting to know your goals, history, lifestyle & budget
We first start with trying understand who you are and what you aim to achieve. If you send us this information in advance, we can make the most out of your consultation time. While most practices aim to offer you one-off solutions for your "problems," we first try to understand the root cause of your problems so we can come up with a long term treatment plan. Understanding your lifestyle and medical history (if you are comfortable disclosing that information) allows us to address not only the cause of any aesthetic issues but also what is possible for treatment plans.

Opt-in Facial (or Body) Optimization Analysis

In addition to discussing what you specifically want to improve, we will also discuss what else could be enhanced. This service is optional, and you will have the option to opt-in (as opposed to opt-out) in your patient intake form.

Discussing the "Menu"
We don't spare the details
Once we know all the details about you and your goals, we can then present a full-course selection of all your options, both surgical and non-surgical. We'll first cut to the chase and tell you our best recommendation, both for best results and for biggest bang for buck. We will also discuss the rationale behind these recommendations, explaining the pros, cons, risks, and other details (with brutal honesty). We will focus only on the options that have been demonstrated to predictably provide the best results. However, if you have any questions on alternate procedures, we will happily address them!

The Game Plan
Numbers, timeline, names, etc.
Your consultation will conclude with a final game plan for addressing your all your concerns in a safe, realistic, and affordable manner. In this plan, we will also include the names of recommended providers, financing options where applicable, and a due diligence checklist for you to use if you would like to keep shopping around for treatment providers. We will also include pre-treatment, post-treatment, and maintenance plans where applicable. Some of these plans may come with product recommendations, but purchasing anything will always be at your discretion. We will always include the both the products we love in addition to the most budget friendly options. Should we have complimentary samples or entry-price discovery kits, we will happily send some to you!
